In-Home Care
Here at Dream Team we provide comprehensive in-home care services that give our clients, your loved ones, the freedom to live in privacy, peace, and comfort. Our caregivers provide quality in-home care that maintains a level of independence that cannot be matched by a nursing home or assisted living facility.
Personal Care
We provide special needs care that can be both short-term or long-term. There are many situations which may require special needs care. Whether people are born with physical limitations or have incurred special needs due to an accident or health issue, we are here to help.

Dream Team is licensed to service Region 9: Tangipahoa Parish, St. Tammany Parish, St. Helena Parish, Livingston Parish, and Washington Parish

SERVICES WE OFFER​
​
1. EPSDT-PCS
a. Program designed for children ages 0-21 with disabilities.
b. Qualified persons receive a minimum of 28 hours per week.
c. To qualify, children must be on Medicaid.
d. To apply, contact us.
2. LT-PCS
a. Program offers assistance with activities of daily living to persons with disabilities ages 21 and up.
b. Qualified persons might receive up to 32 hours per week.
c. To qualify, individuals must have Medicaid.
d. To apply, contact ACS at 1-877-456-1146.
3. Community Choice Waiver
a. Program for persons with disabilities who have Medicaid.
b. Qualified persons receive a minimum of 53 hours per week & there is a waiting list for this waiver.
c. Persons on this program have a support coordination company who visit them on a quarterly basis.
d. To apply, contact ACS at 1-877-456-1146.
4. ADHC Waiver
a. Program offers assistance to persons with disabilities who attend an adult day health care program.
b. Program is similar to LTPCS, but these persons have a support coordination company who visit them on a quarterly basis.
c. Qualified person might receive up to 26 hours a week.

5. Children’s Choice Waiver
a. Program for persons with disabilities ages 0-21 years. There is a waiting list for this program.
b. Qualified persons might receive up to 20 hours per week .
c. Persons on this program have a support coordination company who visits them on a quarterly basis.

6. ROW Waiver
a. Program for persons with certain disability types who are 21 years or older.
b. Max hours received is a case-by-case determination.
c. To apply, contact Florida Parishes at 985-543-4730.
7. NOW Waiver
a. Program for persons with certain disability types prior age 22 & there is a waiting list for this program.
b. Most of these clients receive 24-hr assistance.
c. Persons on this program have a support coordination company who visit them on a quarterly basis.
d. To apply, contact Florida Parishes at 985-543-4730.
8. SIL Waiver
a. Program similar to NOW Waiver, except these clients usually do not have natural supports.
b. No cap on hours might receive, but does have a cap on dollar amount.
c. Require monthly visits & weekly phone calls by Dream Team Supervisors instead of minimum quarterly visits.

9. Supports Waiver
a. Program for persons with disabilities who are on the waiting list for the NOW or SIL waivers.
b. Qualified persons might receive up to 3 hours per week.
c. Persons on this program have a support coordination company.
10. Florida Parishes Program
a. Florida Parishes might provide additional assistance to persons on waiver services.
b. Qualified persons might receive up to 20 hours per month.
c. Respite services available to provide relief for family members.
11. V.A.
a. Program offers assistance to military veterans who need assistance with activities of daily living.
b. Qualified persons may receive up to 6 hours per week.
c. These persons are referred by the office of veteran's assistance.
12. Private Pay
a. We do accept private pay for clients who do not qualify for above listed services and still want PCA services. For more information about private pay, contact us.
